AHCS utilizes the latest remote patient monitoring technology and telehealth in conjunction with good old fashioned house calls. Patients can stay in their homes with their loved ones and out of the hospital. Our number one priority is maximizing our patient’s quality of life, and in good health with as much independence as possible.

Receiving Care From A Familiar Face In A Home Environment

The atmosphere is strange and unfamiliar; not to mention the stress and hassle involved in traveling, particularly for those who are less mobile. We believe receiving care at home from familiar faces provides a comfortable environment, with long-term patient-provider relationships and better patient outcomes. Our providers understand the unique health needs of their patients, which enables them to create custom care plans that maximize our patients’ quality of life. You can take comfort in knowing our team of providers are always here for you.
High Touch, High Care
Through our Chronic Care Management and Remote Patient Monitoring programs the AHCS team is in touch with our patients regularly to ensure they’re maintaining healthy routines. We keep our patients on track and on schedule with taking their prescribed medications at the right times and in with the correct amount of dosages. In addition, when appropriate, or whenever an in-person visit is not possible, we are able to connect with patients remotely through our telehealth platform.
